You definitely have to see the first one to understand the sequel, or else you won't get some of the best BOOM! moments. It's really hard for me to describe the film without giving away the best parts! Let's just say all you think you know about S.H.I.E.L.D. and previous occurrences, you don't... LOL. The action is fantastic, (there is a totally awesome car chase scene that literally kept me on the edge of the theater seat) there are some really funny and totally quotable lines ("on your left!") and the acting was great, too. I really loved how they brought back The Black Widow, (anyone else totally shipping Stevetasha?) and the film has a really great, patriotic message. Captain America is the best!
Oh, and you might wanna stay 'till the end of the credits. Trust me. (and I mean the VERY end)
